1. BEFORE BOOKING MEET & GREET:

  • ​​Review my FAQ page & my Rates & Services page and write down any questions you may have about my services.

  • Please fill out the Dog Profile Form & Client Information Form in their entirety to ensure I can provide you and your Pup with the best possible care & send to me at Derrick@BeachDawgCare.com  before booking the Meet & Greet.  These documents provide me with an in-depth overview of your Dog's history, habits, needs & more.  These two Forms can be filled out electronically & emailed to me.

2. COMPLETE/SIGN & BRING TO MEET & GREET:

  •  Please Download, Print & Complete the Preparing for Your Pet Sitter Form & Permission to Administer Medications Contract & have them with you already completed & signed by all at our Meet & Greet:

*Please List Beach Dawg Care, LLC as the one to administer medications.

*Don't forget to have your Veterinarian sign the Permission to Administer Medications Form.

3. NOTIFY OTHERS THAT I WILL HAVE PERMISSION TO BE ON YOUR PREMISES:

  •    Please Download, Print & Cut Out these Notification Cards.  They will make it very simple for you to notify others that I have permission to be on your premises during my services.  The notification cards have been typeset in postcard format 4 to a page.  For your convenience, you may want to print them on postcard-suitable card stock & have them pre-addressed to the applicable person:

4. AT MEET & GREET:

  • This next Form below will be completed when we first meet and before the first service.  I will have this with me printed out for us to read over and complete together. 


5. AFTER MEET & GREET / ONCE YOU BOOK FIRST SERVICE :

  • Please take a moment & get familiar with these next Contracts/Forms & let me know if you have any questions..

  • I will send these Forms/Contracts to you electronically after our Meet & Greet and before the first booked service is performed so that you can fill out & sign electronically:

Beach Dawg Care is based out of Surfside, Florida & I proudly & exclusively support Dog Parents in:

  • Surfside

  • Bal Harbour

  • Bay Harbor Islands

  • North Beach

*Frequently though, I venture outside these zones, so please don’t hesitate to contact me if you are outside of my Service Area.  There may be a small fee for those beyond my Service Area.
